Comment on: Intracavernosal pressure of the erect penis in children with hypospadias

We perused with interest the research study titled “Intracavernosal Pressure of the erect penis in children with hypospadias”, authored by Ben-Meir and colleagues and published in the International Journal of Impotence Research [1]. The study addresses an important aspect of hypospadias surgery by investigating intracavernosal pressure (ICP) and the relationship of ICP to prior corporotomies. The authors have conducted a meticulous analysis of 61 boys undergoing hypospadias repair, categorizing them into two groups based on hypospadias severity. The findings reveal a significant reduction in ICP during artificial erections, particularly in boys with proximal hypospadias and a history of corporotomies. The study sheds light on a potential association between corporotomies and decreased ICP, which is a crucial aspect for surgeons performing hypospadias surgery.

It is widely acknowledged that the overall incidence of erectile dysfunction (ED) in adults who have undergone congenital hypospadias repair is low, and severe ED cases are rare [2, 3]. However, in a study by Husmann and colleagues, it was reported that moderate to severe ED occurred in up to one-third of patients who had experienced more than one unsuccessful attempt of hypospadias repair [4]. In light of these findings, the study conducted by Ben-Meir and colleagues may corroborate the results of Husmann’s research and potentially pave the way for new avenues of discovery in this field. It could prompt a reevaluation of our existing understanding and knowledge of the subject.
