In 1986, Murry et al.1 discovered a unique way to reduce the size of myocardial infarction in dogs, after occlusion of the circumflex coronary artery. Before occlusion, the researchers subjected the dogs to four intermittent periods of occlusion and reperfusion for five minutes—accomplished through thoracic surgery by snaring and subsequently releasing the vessel with a silk suture. These results suggested that the multiple episodes of angina that often herald myocardial infarction in people may delay cell death after coronary occlusion, allowing better salvage of myocardial tissue through reperfusion therapy. This seminal study introduced the term 'ischemic conditioning', a process that conditions the heart, or other susceptible organs—via mechanical or pharmacological means—to tolerate the effects of acute ischemia-reperfusion injury. The work of Murry et al.1 in dogs set the stage for clinical studies and efforts to identify underlying mechanisms2.
