Transpyloric feeds and bronchopulmonary dysplasia

I read with great interest the article by Wallenstein et al. [1] published in the recent issue of the Journal of Perinatology. They presented the potential benefit of transpyloric feeds (TPF) over gastric feeds (GF) in reducing the incidence of bronchopulmonary dysplasia (BPD). In addition to the limitation of a retrospective study and few other issues as pointed out by the authors, some questions still remained unanswered.

The infants in the TPF group received significantly less mechanical ventilation, 12 ventilator days versus 20 days in GF group. The decrease in the incidence of BPD could merely be a reflection of less severe pulmonary disease rather than use of TPF. Mechanical ventilation is a very well known risk factor for development of BPD.
