We thank Dr. Al-lawama for interest in our recent publication [1] and will address the two points presented.

We are intrigued by the author’s emphasis on the importance of following a systematic approach to introducing a new practice. We acknowledge that attempting early closure of a hemodynamically significant Patent Ductus Arteriosus (PDA) is a popular approach; according to a 2011 survey, two-thirds of neonatologists will always treat these infants [2]. However, we are not aware if such practice is a “standard” of care, as the author described. History taught us that many popular practices were subsequently proven to be inappropriate; for example, the prophylactic use of oxygen to prevent apnea of prematurity was one of these historically popular, but found to be inappropriate, practices.
