Commentary on “Intermediate-term results of TachoSil grafting to correct residual curvature during inflatable penile prosthesis implantation in patients with Peyronie’s disease.”

The intermediate-term data on TachoSil graft (Corza Medical, Westwood, MA, USA) reconstructive surgery to correct residual penile curvature during the inflatable penile prosthesis implantation in patients with Peyronie’s disease (PD) was published in the International Journal of Impotence Research by Pyrgidis and colleagues recently [1]. In this paper, Hatzhichristodoulou and his team reported that 21 out of 25 patients who underwent the penile implant in combination with the sealing (PICS) had total straightness of the penis while the remaining 4 (16%) patients presented with a residual penile curvature of less than 15 degrees following a minimum 24-month postoperative follow-up.

While this paper provides longer-term outcomes on the clinical efficacy and durability of TachoSil graft material, the authors acknowledged several biases and limitations to their study, such as the relatively small number of patients, heterogenous data set, non-blinded report of clinical outcomes, and the lack of a comparative arm. Dr Hatzichristodoulou is widely recognized as the main proponent of the use of TachoSil in penile graft reconstructive surgery [2]. The promising outcomes associated with the sutureless technique using TachoSil have motivated many surgeons to endorse this graft material as a preferred option over other graft repairs. The TachoSil patch and glue technique is considered simple, safe, and effective, but concerns remain regarding its clinical utility in penile reconstructive surgery particularly in non-penile prosthesis implant cases [3].
