Reporting injuries to CQC in primary care dentistry

Dentists will recognise the terms 'near miss' and 'never event' when used in a healthcare context. They will also be familiar with the government and public's concern about safety incidents within NHS Trusts of which there have been many.1,2,3,4,5

Efforts to improve patient safety have largely been focused on secondary care where the majority of acute interventive cases are treated with many different teams delivering that care. However, we shouldn't ignore the significant amount of surgical and operative treatment that is provided in primary care under local anaesthetic where it is still possible for things to go wrong. Indeed, these errors are the subject of extensive research focusing on human factors and training.
