We enthusiastically read the authors’ single-center experience treating oligo-recurrent prostate cancer patients with androgen deprivation therapy (ADT) alone or metastases-directed therapy (MDT) and congratulate them on their potentially practice-guiding findings in areas where no single treatment modality has been established as a standard clinical approach [1]. However, some questions were raised after reading the manuscript.

In oligoprogressive prostate cancer patients, MDT can improve disease control, progression-free survival, and overall survival [2], while also extending the time to secondary systemic therapies. [3, 4] With the advent of functional imaging modalities, the early detection of progressive lesions has increased dramatically. The detection rate accuracy of choline-based PET for prostate cancer recurrence has been demonstrated to be more specific above a PSA value of >1 ng/mL to 1.5 ng/mL [5]. However, in the current study, the range of PSA values at the time of recurrence was 0.1 to 105 ng/mL, which is below the choline PET/CT accuracy limit for detecting recurrence in some patients. As a result, the false-negative nature of choline PET/CT in patients with PSA values less than 1–1.5 ng/mL will obscure the findings of this study. To further elucidate the role of choline PET/CT, the pathological information obtained following surgery, as well as the correlation with choline PET/CT findings in the salvage lymph node dissection group, are critical and will aid in our understanding of the results.
