Comment on: Stratifying the risk of re-detachment: variables associated with outcome of vitrectomy for rhegmatogenous retinal detachment in a large UK cohort study

We read with interest the recent article by Yorston et al. [1]. The authors postulated that the worse prognosis following a vitrectomy for rhegmatogenous retinal detachment in patients under 45 years could be related to the higher likelihood that these patients had an underlying inherited disorder, such as Stickler or Marfan syndrome. We agree that these conditions are challenging to treat and therefore may partly explain the finding. However, they are rare and so perhaps are unlikely to wholly explain this observation.

The estimated incidence of Stickler syndrome is 1/7500 [2], and Marfan syndrome has an estimated prevalence of 1/5000 [3]. Estimates for rhegmatogenous retinal detachment associated with inherited vitreoretinopathy were less readily available. In a survey by Stickler et al. 60% of the 316 participants with Stickler syndrome reported a history of retinal detachment [4]. A survey of Marfan Trust members by Chandra et al. reported that 15.1% of the 185 respondents had retinal detachment [5]. It was unclear what proportion of the cohort aged under 45 analysed by Yorston et al. had an underlying inherited vitreoretinopathy. We suspect the figure would be too low to account for the inferior anatomical success rate of vitrectomy in this group.
