We thank Ahfat for his comments [1]. Due to the word count limitation for the article as well as the large topic of after-cataract, we were not able to supply all the detail such as complete lists of causes, in this instance for anterior capsule opacification. We agree that retinitis pigmentosa and zonular dehiscence are also causes of anterior capsular contraction and sometimes phimosis. We believe that radial relieving incisions of the anterior capsule after lens implantation at the end of surgery is a rather aggressive approach for prophylaxis and would only perform it if significant phimosis had been observed in the contralateral eye. Anterior Nd:YAG laser capsulotomy or, if not effective, secondary surgical capsulectomy would be our preferred strategy.
