Gastric bypass bariatric surgery in patients undergoing allogeneic hematopoietic stem cell transplantation warrants special considerations

Bariatric surgery is an effective treatment for morbid obesity (BMI ≥ 40 kg/m2) with an estimated 220,000 procedures performed in the United States annually. Obesity has no or minimal effect on transplant outcomes [1,2,3,4]. To date, the impact of bariatric surgery on allogeneic hematopoietic stem cell transplant (HCT) outcomes is unknown. Here we present the first case series of bariatric surgery patients undergoing HCT.

Eight patients had a history of gastric bypass bariatric surgery prior to HCT for hematologic malignancies were identified from 2010 to 2018. The IRB of Weill Cornell Medicine approved the study. Acute and chronic GVHD were scored according to the consensus criteria [5, 6]. Malnutrition was defined and diagnosed according to the ICD-10: E43 (unspecified severe protein–calorie malnutrition), or E44.0 (moderate protein–energy malnutrition).
