Right ventricular congestion in preterm neonates with chronic pulmonary hypertension

We read with interest the review by Suresh et al. debating the American Heart Association's [1, 2] recommendations of echocardiography for chronic pulmonary hypertension (cPH) in at-risk preterm infants with chronic lung disease (CLD). The authors eloquently highlight the essential elements of a screening test and demonstrate the shortcomings of echocardiography screening for cPH. Although the authors make strong scientific arguments, and in principle we agree with them, we would like to draw attention to some key considerations that must be taken into account for this population and suggest an interim management approach.
