The search was conducted in early January 2008, with relevant literature included in the review up to a search date of 31 December 2007.

A systematic search of the epidemiological literature was conducted. The search was designed to identify all types of evidence except for case series and case–control studies. Search terms to cover all observational studies, randomized controlled trials and ecological studies were included. The search design also included study types that were excluded from the review at a later stage, after the searches had been completed: cross-sectional studies; studies on growth, body composition and weight during foetal life (including birthweight), infancy and childhood; observational studies with a period of follow-up of 1 year; and studies with outcomes at age <5 years.
