Allo-SCT in a rare t(8;21) evolution of CML

Although clonal evolution with an acquired additional chromosomal abnormality is one of the most common mechanisms of disease progression in CML, t(8;21) with AML1-ETO transcript has rarely been reported in the evolution of CML.1, 2, 3 We present here a case harboring an acquired t(8;21) abnormality in the blastic phase of CML after complete cytogenetic remission from imatinib therapy and demonstrate the successful treatment of this advanced status using allo-SCT.

In May 2006, a 33-year-old man was admitted to our hospital with marked leukocytosis and splenomegaly. Laboratory data revealed a WBC count of 141.6 × 109/l, hemoglobin 107 g/l, and plt count of 345 × 109/l. Leukocyte alkaline phosphatase score was only 2. BM morphology indicated CML in chronic phase with a Philadelphia chromosome. Molecular examination corroborated the diagnosis, showing a BCR-ABL transcript (b2a2 subtype). Imatinib 400 mg daily was prescribed. The patient had a good response and achieved complete hematologic response within 3 months, complete cytogenetic response in 6 months and major molecular response in 12 months (Table 1).
