Reply to Herens et al.

Herens and coworkers in their letter,1 commenting on our last letter,2 basically agree with what we have proposed on the basis of our findings. With respect to our patients, they have analysed a too low number of high-risk MDS patients to retrieve the monoallelic deletion of PI-PLCβ1 gene. In fact, until now, we found the deletion in about one-third of the cases analysed. However, it is important to stress that this alteration, according to our results, could be important in the pathophysiology of MDS, being correlated to a worse prognosis and to a short survival in high-risk MDS patients. Even though the International Prognostic Scoring System (IPSS) could itself be sufficient for defining the fate of such patients, the likelihood exists that IPSS by itself is not sufficient to define the actual prognosis. Stemming from this rationale, it could be interesting to extend the analysis of PI-PLCβ1 gene on high-risk MDS patients to a large number of Haematological Centres. We are keen to share our expertise, samples and probes with regard to this matter.
