We report viral infections after unrelated BMT in 62 children with malignant hemopathies (n = 37), non malignant (n = 6), LSD (n = 15) or CID (n = 4) of whom 39 HLA mismatched (MM) pairs. Conditioning regimen included TBI in 32 cases. Anti lymphocyte globulin (ALG) was used in 46 cases. 34 patients (pts) developed viral infections with 1 (n = 18), 2 (n = 11), 3 (n = 4) or 4 (n = 1) viruses during the first 3 months post BMT: 29/34 pts received ALG and 23/34 were HLA MM. Infection was associated to severe immune deficiency : majority of NK cells, absence or low level of CD 3, CD4 and B cells, low level of IgM. Infections were fatal in 6 cases due to resistant (R) HSV (1 encephalitis), RSV or CMV (2 pneumonitis), ADV (1 fulminant hepatitis), EBV (1 lymphoproliferation, 1 CNS tumor). We treated 2 pts by donor lymphocyte infusion (DLI) (success). (Table)
