Financing graduate medical education

The financial aspect of practicing medicine is typically learned after physicians complete their formal training. In this article, I will summarize the current US system for funding of graduate medical education (GME) and discuss ideas for reform.

In contrast to many other countries that established medical schools through a central source and combined effort, the US medical education system emerged in a disorganized fashion as individual institutions were established. The 129 accredited medical schools, with affiliated hospitals, research programs, and GME programs, embody as many different structures of ownership, curricula, and business plans. Currently, the main contributors to GME financing include Medicare, Medicaid, Department of Veterans Affairs, the Department of Defense, and the Public Health Service.1
