Biotech and bioinformatics, as reviewed by Lengauer et al.1 in their commentary in the December issue, have strongly contributed to the understanding of the predictive relationship between changes in HIV co-receptor usage within patients and the progress of HIV disease. This begs an important question: Why hasn't sequence-based co-receptor prediction followed drug resistance prediction as a routine clinical tool for the management of HIV?
