Pneumoblastoma: a rare lung tumour occurring as a long-term complication after allogeneic haematopoietic cell transplantation

Allogeneic haematopoietic cell transplantation (AHCT) now offers a possible cure for high-risk malignant haematopoietic disorders. Advances in supportive care and GVHD prevention have improved AHCT outcome. Although late relapses do occur, they are rare after 2 years post transplantation. Nevertheless, long follow-up is essential because of the risk of long-term complications. Some of these, such as infections, chronic GVHD (c-GVHD) or solid secondary malignancies (SSM), are life-threatening; others affect patients' quality of life and include mild GVHD, cataract, osteonecrosis and sterility.

Many factors may contribute to the occurrence of second malignancies in the AHCT setting, including cytotoxic chemotherapy, radiation, immunosuppressive therapy, immune stimulation, immune suppression owing to GVHD, antigenic stimulation, oncogenetic virus activity and genetic predisposition.1
