Reply to Prof S Gözdaşoğlu et al

In response to our paper on extramedullary leukemia (EML) in acute myeloid leukemia (AML),1 Gözdaşoğlu et al suggest that we overlooked the importance of orbital granulocytic sarcoma in our patients. The incidence of EML in the setting of AML is not well-defined. It is even more difficult to find the relevant data over orbital granulocytic sarcoma in large series of patients. While it appears that the incidence of this localization is indeed high in Turkey and India, the 7.3% incidence in our patients is not different from those reported in the USA or even Saudi Arabia (2.2% and 8%).2,3 When analyzing the biological characteristics, the group of patients with myeloblastoma was not different from those of the other EML groups in our large retrospective series.

There is indeed no information in our paper on the race or ethnic origin of the patients, as such information is not recorded for any of the patients in The Netherlands. Although socioeconomic status of the patients could be of interest, it was not possible in this retrospective study to obtain those data. Furthermore, it is unlikely that in a fairly homogeneous society such as the Dutch, one should find much correlation between socioeconomic status and the incidence of such a rare manifestation as granulocytic sarcoma.
