Steroid-responsive cauda equina syndrome associated with GVHD after allogeneic hematopoietic stem cell transplantation

Central nervous system GVHD (CNS GVHD) is a rare entity and only 13 possible cases have been reported in the medical literature.1, 2 We report the case of a 43-year-old man with a cauda equina syndrome that we believe is related to CNS GVHD.

Our patient was diagnosed with AML M1 and complete hematological remission was achieved with cytarabine and idarubicine followed by intermediate dose of cytarabine and amsacrine. Second consolidation was by an allogeneic hematopoietic stem cell transplantation from his HLA-identical sister. Conditioning consisted of CY and TBI. GVHD prophylaxis was with in vitro T-cell depletion with CAMPATH-1H followed by T-cell addback and cyclosporine (CsA).3
