New data suggest that patients with steroid-resistant nephrotic syndrome (SRNS) due to primary coenzyme Q10 (CoQ10) deficiency should receive lifelong CoQ10 supplementation. An analysis of retrospective data from 251 patients with CoQ10 deficiency due to biallelic pathogenic variants in COQ2, COQ6 or COQ8B found that oral CoQ10 supplementation was associated with a median 88% decrease in proteinuria after the first year of treatment, as well as improvements in 5-year kidney-failure-free survival, general condition and neurological manifestations compared with untreated patients.
