In the article entitled “Premature ejaculation among men with erectile dysfunction—findings from a real-life cross-sectional study,” Cilio et al. [1] provide further insight on the prevalence of unreported premature ejaculation (PE) in a homogenous cohort of heterosexual men seeking their first medical help for erectile dysfunction (ED). They also compared men with only ED to those with both ED and PE regarding their sociodemographic and clinical baseline features. They concluded that nearly one-quarter of men seeking initial medical attention for ED as their sole complaint also suffered from unreported PE. Patients with concurrent PE and ED were younger, had milder forms of ED, and were more likely to report diminished sexual desire/interest than those with only ED. According to their single center experience, in the past 16 years, the likelihood of assessing patients with both conditions has increased alarmingly among young and middle-aged men, necessitating a tailored therapeutic evaluation [1].

These results are in parallel with the literature. Jannini and his colleagues’ hypothesis suggests that PE and ED are the elements of a vicious cycle in which a man attempting to control his ejaculation intuitively decreases his level of excitation (resulting in ED), and a man attempting to achieve an erection principally attempts to increase his level of excitation (resulting in PE) [2]. A meta-analysis supported this hypothesis by evaluating all available confounders regarding potential causal associations between ED and PE [3]. They demonstrated that PE was associated with a nearly quadrupled risk of ED. A meta-regression analysis revealed that the risk of ED in PE patients increased with age. Intriguingly, they discovered that ED and PE are not distinct entities and should not be rigidly classified as separate diagnostic figures [3]. The existing vicious cycle between the two entities has been proven in other trials, as well [4, 5].
