This paper is novel, important, and contains a vital message that can be paraphrased, the more often an implanted patient is revised, the greater likelihood he has of developing a subsequent infection. In fact, this paper noted a 50% chance with four revisions and 100% infections on patients undergoing their fifth revision. When I reviewed the statistics of our database at the University of AR, which houses almost 4000 implant operations by our group, we note that our findings are similar … Patients with multiple operations tend to get infected more often. An important caveat to remember is that when we began (in 2005) to require that all revision operations included complete component exchange and implant space washout, we lowered the individual case rate of IPP revision infection from 10 to 2%. This paper stimulates us to look at the cumulative rate of multiple operations to see whether our results are as discouraging as this report. Regardless, this paper should stimulate clinicians to prescribe tincture of time with patients who are annoyed by trivial problems such as visible tubing, high riding pump, difficulty with pump usage, etc.
