Abstract
Cardiac troponins provide important prognostic information in patients with acute decompensated heart failure. In a large study by the ADHERE Investigators, 6.2% of 67,924 patients with acute decompensated heart failure were positive for cardiac troponin; these individuals had lower blood pressure and lower ejection fraction than those who were negative for troponin. The study revealed that in-hospital mortality was significantly higher for troponin-positive patients than for troponin-negative individuals (8.0% vs 2.7%; P<0.001). Future studies need to define which treatment changes could improve outcome in troponin-positive patients with acute decompensated heart failure.
