Psoriatic arthritis

Definition

Psoriatic arthritis is a type of seronegative spondyloarthritis characterized by inflammatory arthritis in association with psoriasis. The clinical presentation of psoriatic arthritis is heterogeneous, but often involves the axial joints and distal interphalangeal joints.

    Psoriasis is a chronic, immune-mediated disease that can severely affect patient quality of life. Various kinds of skin manifestations are possible; psoriatic arthritis is a common but underdiagnosed comorbidity. Treatments for psoriasis include topical agents, phototherapy and systemic and biologic agents.

    In this Review, the authors discuss the development of screening tools, outcome measures and new pharmaceutical interventions for managing patients with psoriatic arthritis (PsA). Screening tools are suggested to enable referral from dermatology clinics to rheumatologists for the diagnosis of patients early in the course of disease, and to help identify the best drugs to treat this heterogeneous disease. The authors also discuss the distinctions and similarities of 'tight control' and a treat-to-target approach for the management of PsA.

    Psoriatic arthritis (PsA) is a chronic inflammatory arthritis with a significant genetic component. Here, the authors analyse immune-related genetic markers in 1,962 PsA patients and 8,923 controls to identify novel PsA risk loci and highlight distinct genetic differences between psoriasis and PsA.

    Untreated psoriatic arthritis can have severe long-term consequences, and patients benefit from early intervention. The disease typically presents as psoriasis initially, so dermatologists are ideally placed to manage such interventions, and should coordinate with rheumatologists to ensure optimum patient care. Treatment guidelines and assessment tools from dermatology consortia are among the patient management aspects discussed in this Review.
