Prostate cancer: the right treatment for the right patient

Patients presenting with localised prostate cancer have several treatment options to choose from; however, not all treatment modalities can be applied to each patient. It should also be noted that after treatment with curative intent, up to 30% of patients will develop biochemical failure as demonstrated by a rising prostate-specific antigen (PSA) level. It is important to consider which treatments should be given to these patients, as potentially patients with high risk for biochemical failure could be treated adjuvantly after the initial treatment. This one and a half day meeting was held to discuss these and other issues relating to different treatment modalities for localised, advanced and hormone-resistant prostate cancer. It was held under the auspices of the Dutch Society of Endourology and the European Society of Urotechnology.

Prof Hein van Poppel (Louvain, Belgium) discussed treatment options for cT3 prostate cancer. This life-threatening form of prostate cancer involves perforation of the prostatic capsule (cT3a) or invasion into the seminal vesicles (T3b). The incidence of cT3 disease at presentation is currently in decline compared with an increased incidence of T2 disease. This dramatic stage migration is believed to be primarily due to PSA ‘screening’.
