Acute leukemia is a rare event in the natural history of Waldenstrom's macroglobulinemia, and acute lymphocytic leukemia is even less common.1, 2, 3, 4, 5, 6, 7, 8 We present a patient who developed acute lymphocytic leukemia 22 years after being diagnosed with Waldenstrom's macroglobulinemia.
