Acute myelogenous leukemia (AML) remains fatal for the majority of patients who present with this disease. It accounts for 80% of acute leukemias in adults, and the mortality increases progressively from 0.5 per 100 000 persons in the USA under age 10 years until about 20 per 100 000 persons over 80 years.

Only 5% of patients over 60 years of age are long-term survivors. Since this age group represent the majority of patients with AML, and most such patients are only able to receive a limited amount of intensive chemotherapy, much of the recent focus has been on non-myeloablative targeted or immunotherapeutic approaches for the treatment of older adults with AML. At the same time no effort is being spared in improving the supportive care for such patients. Behind all of these advances is a major effort to expand the understanding of the pathogenesis of the stem cell in leukemia, thus gaining greater insights regarding the etiology of this disorder.
