Neoadjuvant therapy for pancreatic cancer

Advances in surgical technique and chemotherapy regimens have improved the survival outcomes of patients with pancreatic cancer, although these remain dismal relative to most other solid tumours. Attempts to further improve outcomes have led to increasing research interest in neoadjuvant therapy, which is beginning to improve the outcomes of certain subgroups of patients. In this Review, the authors provide an overview of the various neoadjuvant therapy approaches for patients with pancreatic cancer, including discussions of several promising future research directions

Spontaneously evolved progenitor niches escape Yap oncogene addiction in advanced pancreatic ductal adenocarcinomas

It remains unclear how spontaneous Pancreatic Ductal Adenocarcinoma (PDAC) tumors escape Yap dependency. Here, the authors show that lineage plasticity promotes spontaneous relapse following YAP ablation and reveal key transcriptional drivers that overcome YAP addiction in PDAC.

LPAR4 establishes a tumour-initiating niche

Tumour-initiating cells are resistant to challenging growth conditions during the initiation of pancreatic cancer, but how transformed cells acquire a stress-resistant, tumour-initiating state remains unclear. Here, the authors identify a signalling cascade in which LPAR4 induces fibronectin production, cancer stemness and a tumour-initiating niche.

Targeting T cell checkpoints and myeloid suppressor cells is effective in pancreatic cancer

Modulation of T cell immune checkpoints combined with inhibition of chemokine receptors on myeloid-derived suppressor cells can reprogram the highly suppressive tumor immune microenvironment of pancreatic ductal adenocarcinoma (PDAC), and generates durable complete responses in a PDAC mouse model. These results provide a testable clinical regimen for human PDAC.
