APOL1 risk alleles in donor kidneys are associated with graft loss. Using genotyping data of donors and recipients from two kidney transplant cohorts, Zhang et al. show that recipient APOL1 G1 and G2 risk alleles are also associated with increased risk of death-censored allograft loss and T-cell-mediated rejection. Analyses of peripheral blood mononuclear cells revealed high expression of APOL1 in activated CD4+ and CD8+ T cells and in natural killer cells, and enrichment of immune response pathways in carriers of APOL1 risk alleles compared to non-carriers on the kidney transplant waitlist and among healthy controls.
