KEYNOTE-811: pembrolizumab in advanced HER2+ gastric cancer

Interim data for the phase III KEYNOTE-811 trial (NCT03615326) assessing a combination of the anti-PD1 antibody pembrolizumab, the anti-HER2 antibody trastuzumab, and chemotherapy for patients with unresectable or metastatic HER2+ gastric or gastro-oesophageal junction adenocarcinoma have been published. In the double-blind study, 434 previously untreated patients were randomly assigned to either pembrolizumab or placebo, in combination with standard-of-care trastuzumab and chemotherapy. The objective response rate (a secondary end point) in the first 264 patients enrolled was 74.4% in the pembrolizumab group and 51.9% in the placebo group (P = 0.00006). 70.3% and 61.4% of responders in the pembrolizumab and placebo groups, respectively, had a duration of response of ≥6 months, and 58.4% and 51.1%, respectively, had a response duration of ≥9 months. Median response duration was 10.6 months in the pembrolizumab group and 9.5 months in the placebo group. In the cohort receiving at least one dose of pembrolizumab or placebo (n = 433), a similar incidence of adverse events in both groups was observed. Data for the primary end points of progression-free survival and overall survival are awaited.
