Neoadjuvant FOLFIRINOX improves outcomes

Many patients with pancreatic ductal adenocarcinoma (PDAC) are diagnosed with borderline resectable disease; these patients typically have poor outcomes following surgery and adjuvant chemotherapy. In a phase II trial, patients with borderline resectable PDAC received eight cycles of neoadjuvant FOLFIRINOX (fluorouracil, folinic acid, irinotecan, and oxaliplatin) followed by individualized short-course or long-course chemoradiotherapy depending upon the presence or absence of vascular involvement. The R0 resection rate (the primary outcome measure) was 65%; in patients with R0 resection, the median progression-free survival (PFS) duration was 48.6 months and median overall survival (OS) was not reached. For the entire cohort, median PFS and OS were 14.7 and 37.7 months, respectively. These observations merit further investigation in phase III trials.
