The Original Article in the International Journal of Impotence Research “Long-term Histological and Hemodynamic Findings of Repairing Penile Tunica Albuginea Defects with Collagen Fleece in Dogs” describes histological results of the penile tunica albuginea after plaque incision and grafting with the collagen fleece TachoSil (Corza Medical, MA, USA) in an interesting dog model [1].

In this animal model, two adult male beagle dogs 16.9 kg and 16.7 kg underwent experimental surgery at the tunica albuginea of the penis, mimicking a grafting procedure with the use of TachoSil. After exposure of the tunica albuginea, several defects were created through the entire tunica exposing the underlying cavernous erectile tissue. The TachoSil was used to cover the defects. Six months later, the surgical area of the penis was dissected, and histopathological examinations after hematoxylin and eosin staining performed. It was not possible to distinguish the graft site, confirming complete tunica albuginea regeneration. Moreover, no defects were present nor visible. The complete tunica albuginea showed normal and physiological characteristics. There was no fibrosis found. Moreover, the authors performed intracavernous pressure (ICP) measurement before and after grafting with the collagen fleece. Before grafting, the baseline ICP was 9.25 ± 3.9 mmHg. This increased after papaverine injection to 113 ± 31.1 mmHg. Six months after the grafting procedure, baseline ICP was 19.3 ± 2.98 mmHg. This increased to 122 ± 26.1 mmHg after papaverine injection. Although this difference in ICP measurement before and after grafting did not show statistical significance in this dog model, it confirms that the TachoSil is a robust graft that can stand the high ICP even after papaverine injection, mimicking an erection. In summary, long-term histological results of TachoSil grafting in this study showed complete tunica albuginea regeneration with minimal tissue reaction and no underlying cavernous tissue pathology. Additional, hemodynamic results showed robust characteristics of the TachoSil [1].
