Comment on: Changes in aqueous and vitreous inflammatory cytokine levels in proliferative diabetic retinopathy: a systematic review and meta-analysis

Proliferative diabetic retinopathy (PDR) is one of the leading causes of blindness in the world. The PDR involves disrupting the balance of pro-angiogenic and anti-angiogenic factors, leading to neovascularization and its associated vision-threatening complications [1]. The mechanism of regulation and interaction of various cytokines in PDR is complex and has not been fully elucidated. Pro-inflammatory cytokines cause activation of leukocytes, chemokines promote migration of responsive cells, and growth factors such as vascular endothelial growth factors promote angiogenesis [2, 3].

With great interest, we read the article entitled “Changes in aqueous and vitreous inflammatory cytokine levels in proliferative diabetic retinopathy: a systematic review and meta-analysis” published in Eye in 2022 [4]. In this well-designed study, the authors conducted a systematic review and meta-analysis and produced an extensive list of cytokines to illustrate the complexity of PDR pathogenesis. At the outset, we would like to congratulate the authors for writing an informative article with novelty. However, we would like to point out some pre-existing issues with this meta-analysis.
