Recommendations for cytogenomic analysis of hematologic malignancies: comments from the Francophone Group of Hematological Cytogenetics (GFCH)

It was with great interest that the members of the Francophone Group of Hematological Cytogenetics (Groupe Francophone de Cytogénétique Hématologique, GFCH) read the recent article by Rack et al. [1] on recommendations for cytogenomic analyses of hematologic malignancies. We adhere to these guidelines but we would like to provide a few important additions, based on our collective expertize. Indeed, the GFCH has more than 100 active members throughout France and several francophone countries, and serves as the French Hematology Society’s (Société Française d'Hématologie (SFH)) expert group for cytogenetics. Our group has been very active since its creation in 1978, and has great experience of collaborative research (resulting in more than 70 national and international peer-reviewed articles) and national external quality assessments for laboratories. The GFCH’s first set of guidelines was published in 2004 (in the journal Pathologie Biologie) and updated in 2016 (in Annales de Biologie Clinique) by disease experts, each of whom has an international track record in their own field. On the basis of the international guidelines and the medical and scientific literature, we have built decision trees for each malignancy, commonly used in daily practice. Summarizing practice in cytogenetics is always a difficult exercise; the objectives of the present comment are to (i) share our experience in several respects, (ii) emphasize that we consider karyotyping to be the gold standard for most hematologic malignancies, and (iii) confirm that fluorescence in situ hybridization (FISH), single-nucleotide polymorphism arrays and the most recent molecular techniques are very useful complementary methods.

Choice of sample: for myeloid cell cultures: we recommend a peripheral blood culture if bone marrow samples are not available (due to fibrosis) and if abnormal circulating cells detected (blast cells, erythromyelemia, etc.).
