Original studies presenting neonatal growth graphs were published by Babson and Benda in 1976 (J. Pediatr. 89: 814-820). Data for the construction of these graphs were obtained from infants 26 to 40 weeks estimated gestational age born between 1959 and 1963. Since that time, there have been many advances in perinatal and neonatal care. Improved care and the survival of extremely premature infants have necessitated the reassessment of neonatal growth graphs. However, there have been no updated versions of neonatal growth graphs since the 1970's. We proposed that the Babson-Benda growth graphs are not accurate for the neonatal population of the 1990's. We retrospectively analyzed 810 newborn infants born at Oregon Health Sciences University and admitted to the Doernbecher Neonatal Care Center between January 1, 1991 and December 31, 1995. Mean birth weight, length, and head circumference were compiled with respect to gestational age. Estimated gestational age was determined utilizing the last menstrual period, ultrasound dating, and Ballard assessment by the attending neonatologist. Smoothed percentile curves for mean birth weight, length, and head circumference were generated and compared to the data and curves presented by Babson-Benda. Our data revealed that for mean birth weight, length, and head circumference, there was no significant difference between our curves and the Babson-Benda curves for infants between 28 and 40 weeks estimated gestational age. At estimated gestational ages less than 28 weeks, there is no significant difference, but the trend showed considerable deviation between the two sets of curves. Between 23 and 26 weeks estimated gestational age, there were no data from Babson-Benda comparable to our curves. We conclude that the Babson-Benda curves are accurate for the neonatal population of the 1990's for infants 28 to 40 weeks estimated gestational age. In addition, we present an updated graphical representation of mean birth weight, length, and head circumference for infants 23 to 27 weeks estimated gestational age.
