Very low achieved SBP increases risk of cardiovascular death

Current guidelines recommend a blood-pressure target of <140/90 mmHg to reduce cardiovascular risk, but the lower boundaries of this target have not yet been defined. A secondary analysis of the ONTARGET and TRANSCEND trials involving patients with a history of cardiovascular disease now reports that a systolic blood pressure (SBP) target of <120 mmHg is associated with increased risk of cardiovascular and all-cause death, but not myocardial infarction or stroke. This finding indicates that the lowest possible blood pressure is not necessarily the optimal target for reducing cardiovascular risk in this high-risk cohort.
