Ticagrelor not superior to clopidogrel for PAD

Evidence from previous trials in patients with other cardiovascular diseases suggest that ticagrelor might have increased benefit in patients with PAD compared with clopidogrel. To test this hypothesis, the EUCLID investigators randomly assigned 13,885 patients with symptomatic PAD to receive monotherapy either with ticagrelor or clopidogrel. Patients had to meet one of two eligibility criteria: an ankle–brachial index (ABI) ≤0.80 or previous lower extremity revascularization. All patients underwent genotype testing for CYP2C19 and patients with two loss-of-function variant alleles — a genotype associated with poor metabolism of clopidogrel — were excluded.
