Population-based analyses among 184 patients diagnosed with large granular lymphocyte leukemia in the Netherlands between 2001 and 2013

We read with interest the article by Marchand et al. on the value of hematopoietic stem cell transplantation for the management of T-cell large granular lymphocyte (T-LGL) leukemia.1 The World Health Organization classification of 2008 recognizes two chronic LGL proliferations that are very rare, namely T-LGL leukemia and chronic lymphoproliferative disorder of natural killer cells (CLPD-NK);2 the former being the most common entity3 and the latter being a provisional entity.2 Both entities have similar clinical and biological characteristics, as well as response to therapy.3 In addition, they are associated with favorable outcomes.3, 4 Therefore, the vast majority of LGL patients will usually not require intensive treatment with hematopoietic stem cell transplantation.5 Other studies than that of Marchand et al.1 are thus needed to provide information on the general LGL leukemia population at large. At present, only two multicenter, population-based studies on LGL leukemia are published that are based on patients reported to specialized LGL leukemia registries from France, Italy and the United States.3, 4 However, due to their study design, those studies may be prone to selection and/or referral biases. Hence, studies including all patients within a well-defined geographical area are needed to diminish such biases and to complement existing population-based studies such as those from Marchand et al.1 and the former two multicenter studies.3, 4

Here we report the outcomes of a nationwide population-based study on incidence, primary treatment and survival among 184 newly diagnosed LGL leukemia patients in the Netherlands.
