Vaccine for IDH mutant glioma

Isocitrate dehydrogenase 1 (IDH1) mutations — particularly R132H — are common in diffuse gliomas. Now, Platten et al. report that the first-in-human trial of an IDH1-R132H-specific peptide vaccine (IDH1-vac) in 33 patients with IDH1-R132H+ astrocytomas met its primary end point, demonstrating safety and immunogenicity. Vaccine-induced immune responses were seen in 93.3% of patients, with 3-year progression-free and death-free rates at 0.63 and 0.83, respectively. Pseudoprogression frequently occurred, indicative of intratumoural inflammatory reactions. Patients who did not mount an IDH1-vac-induced immune response showed disease progression within 2 years.
