A case of severe cutaneous, GI and liver GVHD in a patient with multiple myeloma, status-post-second auto-SCT

We read with interest the report of WR Drobyski et al. (January, 2009) on ‘Severe autologous GVHD after hematopoeitic progenitor cell transplantation for multiple myeloma’ in which the authors report life-threatening GVHD in five patients who underwent autologous transplantation for multiple myeloma. We report an additional case of severe, life-threatening GVHD following auto-SCT for multiple myeloma, with skin, liver and gastrointestinal (GI) tract involvement, documented by characteristic histology at all involved sites.

A 61-year-old female with IgG-κ multiple myeloma diagnosed 6 years before and treated with 3 courses of chemotherapy underwent her first auto-SCT 22 months before presentation. She relapsed 12 months after her first transplant, and was initiated on lenalidomide conditioning in preparation for her second auto-SCT. She was transferred to our hospital on day 41 status-post her second auto-SCT.
