In the Mosaic creation narrative, one of the tasks that God gives Adam in the Garden of Eden is the naming of all living things – it is good to know what kind of beast one is dealing with! In the present issue of LEUKEMIA, Streubel et al.1 coin the name for one member of the heterogeneous group of not-otherwise specified peripheral T-cell non-Hodgkin's lymphoma. In terms of the identification of hematological malignancies, the paradigm that has emerged over the past 50 years since the initial description of the Philadelphia chromosome in chronic myeloid leukemia (CML)2 is that each subtype of malignancy may be identified by a specific spectrum of cytogenetic abnormalities, and, in many instances, specific chromosomal translocations. Malignant clones may be morphologically and immunophenotypically identical, but associated with widely different prognosis due to the underlying genetic damage. Many chromosomal translocations are thought to be pivotal to the pathogenesis of the malignant clone and are therefore ideal tumor-specific targets for therapy, a concept that has been realized with the development of inhibitors of the BCR–ABL fusion kinase that underpins CML.3
