Long-term remission in a patient with hepatosplenic γδ T cell lymphoma after cord blood stem cell transplantation following autologous peripheral blood stem cell transplantation

Hepatosplenic γδ T cell lymphoma (HSγδTCL) is very rare, representing <5% of all peripheral T cell lymphomas. Affecting mainly young men, HSγδTCL is characterized by pronounced hepatosplenomegaly without lymphadenopathy.1 Despite a broad array of potential therapeutic approaches, HSγδTCL is very aggressive, with a median survival duration of <2 years.2, 3 We report successful treatment with an unrelated donor cord blood transplantation (UCBT) following autologous peripheral blood stem cell transplantation (APBSCT) in a young woman with HSγδTCL.

A 25-year-old woman was admitted to our hospital because of massive splenomegaly (extending 15 cm below the left costal margin) and hepatomegaly (extending 5 cm below the right costal margin). The peripheral blood cell count was WBC, 2900/μl; Hb, 8.0 g/dl; and plt, 92 000/μl in a bone marrow aspirate, 12.8% of cells represented an infiltrate consisting of medium-size lymphocytes with pale cytoplasm and irregularly shaped nuclei without prominent nucleoli; the immunophenotype was CD2+ (99.2%), CD3+ (95.6%), CD7+ (94.5%), CD4+ (33.7%), CD5− (4.6%), CD8− (10.0%), CD19− (10.3%), CD56− (1.7%), HLA-DR+ (80.2%), TCR-αβ− (2.6%), and TCR-γδ+ (95.2%). Cytogenetic analysis indicated a normal karyotype; TCR-γ gene rearrangement was detected by PCR analysis.
