Rituximab and CHOP-like chemotherapy in good-prognosis diffuse large-B-cell lymphoma

DLBCL is the most frequent lymphoma, generally diagnosed in adults over 40 years of age. The CHOP regimen, introduced in the late 1960s, improved outcome of patients with DLBCL, and was curative in some cases. Another major breakthrough was the presentation by the Groupe d'Etude des Lymphomes de l'Adulte (GELA) of the results of treatment with a combination of CHOP and rituximab.1, 2 A 15% higher survival rate for patients treated with a combination of CHOP and rituximab compared with those treated with CHOP alone was maintained over 5 years.2 The results of the MabThera International Trial (MInT) in young patients also show a major improvement in PFS and OS when rituximab is added to different CHOP-like regimens. The MInT included young patients with good-prognosis disease according to the IPI (over 70% of them with a localized tumor), and clearly demonstrated a benefit of CHOP-like chemotherapy plus rituximab, similar to that seen for older patients in the GELA study.
