Clinicians frequently face the question of how to begin pharmacologic therapy in patients with CHD and associated low HDL and LDL levels, especially if the LDL concentration falls below the threshold for LDL-lowering drug therapy according to current guidelines. Should one focus on raising HDL or on lowering LDL?
