What is the most accurate test to differentiate pancreatic cystic neoplasms?

Cystic lesions of the pancreas are a diagnostic challenge for clinicians. Owing to the increasing detection of such lesions, as a result of more sensitive abdominal imaging modalities, and the more liberal use of these tools, the need for accurate diagnosis has become even more pressing. The critical question is how to differentiate a non-neoplastic cyst/pseudocyst from a neoplastic cyst, and whether the latter is benign or malignant. The most commonly encountered cystic neoplasms include serous cystadenomas, which have an exceedingly low malignant potential, and mucinous cystadenomas, which have a variable but clinically relevant risk of progressing to malignancy.1
