Hyperphosphataemia in CKD

Renal excretion of dietary phosphate has an important role in the maintenance of phosphate homeostasis. In patients with late-stage chronic kidney disease (CKD), nephron loss and the inability of adaptive mechanisms to maintain phosphate excretion lead to phosphate retention. The resulting hyperphosphataemia is associated with increased cardiovascular risk in these patients. Here, Tilman Drüeke and Pieter Evenepoel outline the pathophysiology of hyperphosphataemia in CKD and the potential adverse effects of excess phosphate on the heart, bone and vasculature.
