Migraine sufferers experience pounding headaches, which sometimes are preceded by a visual aura. Now Bolay et al. show that cortical-spreading depression, the cause of the aura, activates trigeminal afferents, which act to cause inflammation of the pain-sensitive meninges, generating the headache.
