What is the prognostic value of the pT3a tumor classification for renal cell carcinoma?

Our understanding of the significance of renal fat invasion in RCC is evolving, and this study represents an important contribution to the debate. Some physicians have advocated revision of the current AJCC classification of pT3a tumors, because of a lack of evidence for the prognostic significance of renal fat invasion.1, 2 The controversy over revising the pT3a classification is brought into focus when examining small pT3a tumors (<4 cm), which would be classified as pT1a if renal fat invasion were removed from the definition; patients with pT1a tumors have an excellent prognosis compared with patients with pT3a tumors.
