Prophylaxis for prevention of PCP is recommended as standard of care for patients with a diverse range of conditions including advanced HIV infection and many malignancies, as well as for patients undergoing HSCT or solid-organ transplant. Without effective chemoprophylaxis the incidence of PCP can be high in these populations. For example, PCP occurs in over 20% of patients with acute lymphoblastic leukemia,1 and can cause a mortality rate of nearly 60%.2 Over the past 20 years, however, the use of prophylaxis against P. jirovecii has substantially decreased the incidence of PCP in at-risk populations.
